Add butter in a bowl of a stand mixer and beat on low-speed until it becomes creamy and smooth. If you don’t have a stand mixer, you can also use a large bowl and an electric mixer. Make sure to scrape the sides of the bottom of the bowl.
Add 2 cups of powdered sugar, cocoa powder, vanilla extract, 1/8 teaspoon of salt, and 3 tablespoons of heavy cream and mix on low speed. Stir until well-combined.
While the mixer is on low speed, gradually add the remaining powdered sugar. It will look dry.
Add remaining heavy cream or milk, into the mixture, one tablespoon at a time. Start the mixer on low-speed, then gradually increase speed to medium high. Add additional tablespoons of cream until you have a smooth, pipe-able consistency.
Pipe the icing over your cupcakes or frost your cupcakes immediately.
