Pour all the milk into the crockpot, then add the condensed milk, thickened cream, vanilla extract, and semi-sweet chocolate chips. Stir everything together, then get into the bottom and corners, and keep stirring until the thick and thin ingredients are well combined. Once mixed through, cover the crockpot with the lid, then set it on high, and allow the mixture to cook for about 2 hours. Stir the mixture every half hour to make sure that the chocolate chips dissolve into the liquid, then after the 2 hours are up and the chocolate is fully combined, turn off the crockpot, and make sure not to overcook the hot chocolate because the milk will curdle. Ladle the hot chocolate into mugs or cups, then serve, and enjoy!
